To apply for membership, complete the form below including your CV for Full and Associate Membership, or your Proof of Current Enrolment document and the length of time and involvement in the field of ADHD for Student Membership.

If your application is successful, you will receive an email within 2 business days of submitting your application with payment information. Please contact us if you have any problems submitting your application and we look forward to welcoming you into AADPA.
