In children and young people, ADHD can sit behind anxiety, low mood or substance use, and knowing when to look further matters.

When a child or young person presents with anxiety, low mood or risky behaviour, it makes sense to address what’s most visible first. But when progress stalls, or some symptoms persist despite good support, clinicians may need to step back and ask whether ADHD is part of the picture. Assessment becomes more complex again when substance use, emotional dysregulation and emerging mood symptoms overlap, and when the order and safety of treatment carry real weight.

In this ADHD Case Rounds session, Consultant Psychiatrist Dr Matthew Boulton and Developmental and Behavioural Paediatrician Dr Reetika Dhir come together for a multidisciplinary discussion of two real-world clinical cases. One is a primary school-aged child whose anxiety has improved but whose hyperactivity hasn’t. The other is an adolescent whose substance use complicates both diagnosis and treatment.

Together, Drs Boulton and Dhir will unpack the signs that call for a referral, how to tell overlapping presentations apart, and what to put in place before and during treatment.
